Eric is an ordinary guy on an extraordinary journey. He is a young man starting to think about a home of one’s own. Eric and his parents have worked with a P4P facilitator through the Individualized Housing Workshop program to begin to create a housing plan for when Eric is ready to live in a home of his own.
Moving out on one's own is an extraordinary moment for any young person, it is one of life’s biggest transitions. This housing plan is helping Eric think about what he would like his home to look like while also helping Eric’s parents think about the support and steps they need to take in order to help Eric set his plan into action. This plan is helping Eric and his parents consider the many many possibilities in front of him!

Independent Facilitation is an ongoing process that supports an individual with a disability to develop a vision for their future and to take the steps necessary to work towards their dreams. Facilitation involves a trained Facilitator working alongside a Person/Family over time. It involves expanding personal networks, planning and connecting people in community. In this instance an Independent Facilitator worked with Eric and his family to help Eric develop his vision for a home of his own. While the plan created together is only a reflection of a moment in time, Eric and his family will continue to work with their facilitator to put the plan into action.
